Summer is right around the corner! So even though school is still in session, it’s not too early to start gathering those camp physicals, sports physicals, shot records, and any other documents needed to get ready for the NEXT school year.
How do I get my child’s physical form completed for camp or sports?
As everyone knows, check-ups can get booked out in just a few weeks, so it’s important not to wait until the last minute to schedule. Please call our office at 501-664-4044 to see if it’s time to schedule the next wellness visit, and don’t delay getting it on your calendar. If your child is due for a check-up, we can complete any physical forms needed at that time, as well as provide an up-to-date shot record.
If your child has had a check-up in the last 6-9 months, we may be able to complete the camp or sports forms based on that visit note. You can fax the physical form to 501-664-4064 or email it to our medical records staff here. Please allow 5-7 days for completion. If the provider determines your child DOES need to be seen, we will reach out to you. In many cases, a copy of the visit note is acceptable in lieu of an actual physical form.
How do I get a copy of my child’s shot record?
Our medical records staff can provide records in various ways, such as secure email, patient portal, or regular US mail. You are also welcome to pick them up at our front desk. Please request shot records by one of the following methods, making sure to include the child’s name, date of birth, and a working phone number that will be answered.
1. Call 501-664-4044, ext. 231, and leave a voice mail.
2. Email your request to our medical records coordinator here.
3. Use the Contact Us feature on our website.
4. Fax a request to 501-664-4064.
5. Send a request via the patient portal.
What if my child’s shots are not up to date?
Schools do require that children be current on all routine childhood immunizations. If your child is behind on shots but up to date on their check-ups, please call us at 501-664-4044 to schedule a nurse visit. Otherwise, shots can be given during the check-up. What about my child’s ADHD meds?
As most parents know, children on ADHD medication need a med check every 6 months. We encourage parents to make sure med checks are up to date well before school starts to ensure a good start to the new school year. Some of our providers do offer telemedicine visits for med checks.
A Word About Inhalers and Asthma Action Plans
If your child needs a new inhaler or an updated asthma action plan, please call our Nurse Line at 501-664-4044, ext. 244. She will review your child’s record and determine if they need to be seen prior to renewing the prescription. If your child’s symptoms have changed, we recommend scheduling an office visit for an asthma check.
